	Standard Application Form - you must complete one of these in order to apply for either Full or Youth Equity membership. PDF version
	Rejoin Application Form - if you want to come back into the union having previously been a member, you must complete this. PDF version
	Student Application Form - if you are currently studying on a higher education full time course in performance or a related subject lasting one year or longer, this is the form you need. PDF version
	Graduate Application Form - if you have graduated in the past 12 months from an NCDT or CDET accredited course, or from a conservatoire singing course or theatre design course or from the RAM's Musical Theatre Course, you should complete this form. PDF version
