Form - Stage Equipment Indemnity - Multiple Performers

This is a downloadable pdf form that duos, and other multiple performers can download, print and ask venues to sign them prior to their stage performances. It is derived from a suggestion by Branch member Ricky Garrett and we think that there is a great need for such an agreement in today's increasingly electronic age so we have included them here. There is also a similar form for single (solo) performers. It's work in progress so if anybody has any ideas or suggestions...
